*All Volunteers who interact with persons receiving TEFAP or have access to their records must complete and sign Level 1. TEFAP leaders must also sign level 2. (Level 2: sign both places before inserting your email). The program will ask for your email address to confirm you are a real person. You must have access to your email account to verify your email address. You will receive a copy of the document once it is complete. If you do not receive a copy, the process was not complete and must be redone. Thank you for your patience.
